Very nice VIZ Isotap WP-26A variable isolation direct isolated transformer.  Tested and working as designed.  400 VA power rating and 105-130V primary voltage rating.  The ground lug was broken off of the molded AC plug so I replaced it with a heavy-duty 3-prong plug.  I will also include a replacement AC cord with molded 3-prong plug should that be your preference.  See photos for more details.
